Attached is a standard engagement letter with our fee schedule for PHI Medical Records LLC. Please sign the engagement letter and return via email or fax. Once received, we can begin work on your records requests.

PHI HIPAA FORM
The Attorney General of Texas created a universal HIPAA form. We request you fill out the top section of the form and have your client sign/initial this form and return it to us for our use. A pdf version is available for download. Some adjustments have been applied.
