Attached is a standard engagement letter with our fee schedule for PHI Medical Records LLC. Please sign the engagement letter and return via email or fax. Once received, we can begin work on your records requests.
Worried about space or handwriting? Follow this link to enter the client's information and have a completed HIPAA form immediately emailed to you. Please have the client sign/initial and send it back to PHI via email or fax. Once we have received this form with a list of providers we can begin the requesting process.
PHI HIPAA FORM
The Attorney General of Texas created a universal HIPAA form. We request you fill out the top section of the form and have your client sign/initial this form and return it to us for our use. If you prefer to type in answers, an electronic form of the HIPAA is included below.
